1. Orientation (Total: 10 points)

  • What is the current day of the week? (1 point)
  • What is the current time? (within one hour) (1 point)
  • What is the name of this place? (1 point)
  • What city are we in? (1 point)
  • What state are we in? (1 point)
  • What country are we in? (1 point)
  • What season is it? (1 point)

2. Immediate Memory (Total: 3 points)

  • Repeat the following words: Apple, Table, Penny (1 point for each correctly repeated word, immediate recall)

3. Attention and Calculation (Total: 5 points)

  • Serial 7s: Starting from 100, subtract 7 and keep subtracting 7 (e.g., 93, 86, 79, etc.) (up to 5 points)
    • Alternatively, spell “WORLD” backward (D-L-R-O-W) (5 letters, 1 point for each correct letter)

4. Recall (Total: 3 points)

  • Recall the three words from the immediate memory section after a delay (1 point for each correctly recalled word)

5. Language (Total: 9 points)

  • Name these objects: Watch, Pen (2 points: 1 point for each)
  • Repeat the following phrase: “No ifs, ands, or buts” (1 point)
  • Follow a three-step command: “Take a paper in your right hand, fold it in half, and put it on the floor” (3 points: 1 point for each correct action)
  • Read and obey the following written command: “Close your eyes” (1 point)
  • Write a sentence (1 point for a sentence that contains a subject and a verb)
  • Copy a design (intersecting pentagons) (1 point)

Interpretation

  • 25-30: Normal cognitive function
  • 21-24: Mild cognitive impairment
  • 10-20: Moderate cognitive impairment
  • 0-9: Severe cognitive impairment
